How do I find out how much my Vacation benefit will be?
You may contact the Fund Office at (602) 249-3582 or at 2400 W. Dunlap Avenue, Phoenix, AZ 85021 after November 1 for the upcoming December 1 distribution.
How is my Vacation Benefit calculated?
A designated amount per hour worked will be contributed by your Employer to your Vacation Benefit account in accordance with the Union agreement. Your account will also be adjusted proportionately for that year’s interest income and administrative expenses.
How do I apply for my vacation benefit?
You do not need to apply for your Vacation Benefits. Your benefit will automatically be distributed on the first business day of December and will be mailed to the last known address on file.
How do I change my address?
Complete and print the Online Change of Address Form and mail or deliver to the fund office at 2400 W. Dunlap Avenue, Phoenix, AZ 85021. Each participant is responsible for keeping the Fund office apprised of any change of address.
When do changes need to be received in order for my next yearly benefit to be updated?
Changes must be received no later than November 10 in order to be included in the December 1 Distribution. However, it is recommended that you notify the plan immediately of any changes, such as your address.
Can I have my Vacation Benefit deposited to my account instead of receiving a check?
Direct Deposit is not available for Vacation Benefits at this time.
What happens to my Vacation benefit if I do not receive it?
In general, you have up to 3 years to claim your vacation benefit. If left unclaimed for at least 3 years after the distribution date, the funds will be forfeited back to the Vacation Fund.
How can I find out if I have past Vacation benefits due to me?
Please verify that your name is on the Benefits Due List. If so, please complete and print the Online Check Tracer Form and mail or deliver to the fund office at 2400 W. Dunlap Avenue, Phoenix, AZ 85021
Can I fax information to the fund office?
The fund office can only accept documents with original signatures to prevent fraudulent activity.
